Once you have selected a PICOT question discuss your strategies for conducting a systematic search/review of the literature in order to answer your question. • Citations: At least one high-level scholarly reference in APA format from within the last 5 years
PICOT Question: In elderly patients age 65 or above with congestive heart failure. How effective is post discharge intervention focusing on holistic nursing care compare to non -holistic nursing interventions affect the hospital readmission rates within 30 days post discharge?
P-elderly patients age 65 or above with congestive heart failure
I-post discharge intervention focusing on holistic nursing approach such as home care. This will incorporate Jean Watson’s theory of human caring as a framework to assess the patient’s situation in developing appropriate care plans.
C- post discharge patient who receive post discharge intervention such as remote monitoring these includes, telephone calls, tele monitoring, and follow- up visit in clinics
O- Reducing the rates of hospital readmission for patient with congestive heart failure who received post-discharge holistic nursing approach.
T- within 30 days post discharge.
