Diabetic prevention and control program
The purpose of the diabetic prevention and control program will be to prepare the health professionals in the hospital and the community members on Ambulatory Diabetics and training on diabetes self management . The participants will have the opportunity to discuss and review treatments on diabetes and to discuss clinical management, learn about diabetes, follow-pup skills, assessments skills, review various components on the curriculum and examine behavior and psychosocial change issues.
Lesson plan
Day 1: overview of the lesson
The participants will be informed about Diabetes prevention and control. By the end of the lesson the participants will be able to know the various components of DSMT and ADEF program. They will study the prevalence of diabetes
Day 2: Diabetes resources:
By the end of the lesson the participants should be able to describe the procedures used in diabetes prevention and control program. These include the various interventions used to help reduce mortality and morbidity causes by diabetes
Day 3: clinical overview and management
The participants should be able to describe the clinical treatments and management of diabetes. The participants will learn of the pedagogical techniques which are crucial for all the educators of diabetes. They will learn the processes and theories will they will apply in their practice. This will also mean teaching them on ways to become excellent communicators.
Day 4: medical nutrition therapy
Participants will learn on the best nutrition fit for patents suffering from diabetes. This will also include teaching on lifestyle change. Participants will be offered theoretical framework which are directly related to efficient and practical control with the aim of positively influencing the results of a diabetic.
Day 5: physical activity
By the end of the lesson participant will be able to learn the importance of physical exercise. The participants will be required to learn the potential benefits of active lifestyle to a patient by increasing their fitness, improving the levels of lipids, glcemic control improvement and for reducing the risks of cardiovascular disease.
Strategies to be used in teaching the ADEF/DSMT program
This course will take two important approaches: first is that the health professionals will undertake the modules on Diabetics education which was established by the International Diabetes Federation .The second approach will target the community heath promoters who will receives “the Road to Health Tool kit” program which was established by the National Diabetics Education Program. All these methods will use English language. The structuring of the course is as follows: on the first day, both the health practitioners and the community members will be trained together on the overview of the diabetes control and prevention program. The second and third day will be separate courses for the two groups where they will be provided with related information but it will be different based on the two methodologies selected. The heath practitioners receiving the training will be the nurses, physicians, diabetes and health educators, psychologists, dentists, pharmacists, dietitians and social workers.
The attendees should be border residents with a job experience of at least one year in the diabetes field. The health practitioners should also be a leaders in this filed or serves as a members of an interdisciplinary club, should have teaching and mentoring skills and one who is committed to actively work in the community as well as in the hospital in the filed of diabetes. The health practitioners should him professional affiliations, excellent in networking and communication and have the capability of developing training (Pan American health organization 2009).
The community health promoters have to be residents in the region , be interested or are working in the area of diabetes, are capable of developing a bridge between the health care system and the members of the communities, they can promote and provide health education which is culturally appropriate , are committed to help others, have communication skills and should use the education methods which are popular, advocate for communities and individual and should be able to provide socially support and counseling. They should also be able to produce training.
The best strategies to use in teaching this program depend on the topic of the day. Activities are supposed to be individual in the program such as telling the participants to write what their expectations are, what they already know about diabetes and what thy want to learn. The program will also use PowerPoint presentations to presents the objective of the program, and goals of the training. Another effective method of training is the use of play role and dramatization. There the participants will be required to play the role of the William or the Cruz’s family story as away of introducing participants on diabetes prevention and the study of DDP. Concerning the topic the participants will study the flipchart kit on the story of Williams and Cruz family. In teaching self monitoring skills on diabetes the participants will be required to revise the glucose gauge so that they will be able to know how the tool is practically used and to interpret the results about the blood sugar level.
The best teaching strategy on the relationship between diet issues and diabetes w is to first enable the participant’s e explains what calories are all about and where it comes from. The participants will be required to work in groups and to study the calorie and fat counter and what they interpret from the kit. They will draw flip charts on fat detectives, portions and on the Cruz family. Finally they should come up with a food plan which recommends the best diet for individuals suffering from diabetes.
In the area of physical exercise participants will learn and practically carryout movements for their health and the required movements for the health of individuals suffering from diabetes. They will develop ways on lifestyle change and engage in reinforcing knowledge and sharing experiences. A video will be shown of the physical exercise which the participants will, be required to follow.
