Identify and discuss barriers that contribute to the COPD patient’s quality of life and medical care/treatment. Discuss barriers in achieving comfort for patients with dyspnea.
Describe emotional issues that may be present. Explain your intervention(s) to minimize distress. Include an educational plan for the COPD patient and his or her family. Describe evidence-based interventions you will include in plan of care.
Discuss how the IPT can facilitate goals of care for dyspnea and related symptoms. Include how the IPT contributes to the family care and care needs. How will you determine intervention(s) for maximum outcome potential?
