Chief Complaint
Ms. Adler injured her right wrist in a motor vehicle accident caused by driving under the influence of alcohol. She also expressed passive suicidal ideation and is considered a self-harm risk.
History of Present Illness
Rachel Adler, a 20-year-old White woman with a history of alcohol use and anxiety, was brought to the ER by law enforcement after a motor vehicle accident where she was driving under the influence of alcohol and injured her right wrist.
Assessment in the ER confirmed a sprained right wrist, and her blood alcohol level was measured at 0.2%, which is above the legal limit for individuals under 21. Ms. Adler reported not knowing what happened but was informed that she received a ticket for DUI. Upon arrival, she reported her pain level as 8/10, and pain relief was noted after receiving ibuprofen. She denied experiencing any nausea or vomiting. Based on a positive assessment for self-harm, she was admitted to the adult mental health unit.
Allergies
Ms. Adler has a peanut allergy, and there is no known drug allergy (NKDA).
Past Medical History
Ms. Adler has no significant medical issues and denies the use of recreational drugs and tobacco.

Ms. Adler takes Ortho Tri Cyclen Lo, one tablet orally daily at 0800, and occasionally uses ibuprofen for menstrual cramps.

Her mother has an anxiety disorder, and her father has no major medical issues. Her maternal grandfather had a history of alcoholism.
Social History
Ms. Adler is a junior college student who is single and not currently sexually active. She denies tobacco and illicit drug use but acknowledges using alcohol at least three times a week.
Thought Content
Ms. Adler displays passive suicidal ideation and expresses a lack of concern for her own life. Although she does not have active suicidal ideation or specific plans to end her life, she feels distressed due to school and her relationship with her mother. The circumstances surrounding her DUI incident have deeply affected her emotionally.
Perceptual Disturbances
There is no evidence of delusions or abnormal tendencies observed during the examination. Ms. Adler does not experience hallucinations or false sensory perceptions.
