A plan of care is a presentation of information that clearly describes the services and support being provided to a person. Care plans should be developed and agreed upon with the person on whose behalf they are focused as part of the care planning and review process.
A care plan is made up of individual records of care, which then contribute to the overall plan of care for a person.

They not only help define the roles of support and care workers in providing consistent care, but they also allow the care team to tailor the level and types of support for each person based on their individual needs.
