Case 11 A 30-year-old male is brought to the emergency department in an ambulance after his partner found him on the floor in their home. The patient is drowsy on admission, and so a collateral history is obtained from his partner. She states that she walked into the bedroom and noticed the patient on the floor. He appeared to be unconscious, and his limbs were jerking. She noted that he had been incontinent of urine. She reports that the patient has had one previous episode like this before. On examination, the patient is drowsy but rousable. His eyes open to voice and he is able to move his limbs on command. He is able to mutter some unintelligible sounds. Neurological examination reveals globally increased tone and brisk reflexes. On examination of the oral cavity, the tongue appears bruised and swollen. What is the likely cause of these symptoms?
